OCR description
The business associate (BA), ProComp Software Consultants, reported that one of its covered entities (CE) was the subject of a cybersecurity incident that affected the protected health information (PHI) of 1,008 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and diagnoses. The BA notified HHS, and the CE notified the affected individuals. In response to the breach, all staff were retrained.
