New Bedford Jewish Convalescent Home, Inc.

The covered entity (CE), New Bedford Jewish Convalescent Home, reported that an employee was the victim of an email phishing scheme that affected the protected health information (PHI) of 873 individuals. The PHI involved included names, dates of birth, addresses, dates of death, diagnoses, medications prescribed, Social Security numbers, drivers’ license numbers, claims and financial information, health insurance information, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on the proper methods of identifying fraudulent email communications.
