OCR description
The covered entity (CE), Family Health Services MN dba Entira Family Clinics, reported that its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 1,975 individuals. The PHI involved included names and Social Security numbers. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the BA implemented additional administrative, technical, and security safeguards to further protect PHI.
