The covered entity (CE), Walmart, reported that its business associate suffered a breach that compromised the protected health information (PHI) of 2,071 individuals. The PHI involved included names, addresses, dates of birth, telephone numbers, medication information, and health insurance information. The CE notified HHS, affected individuals, and the media.
