OCR description
The covered entity (CE), Arizona Oncology Associates, reported that an employee lost documents that contained the protected health information (PHI) of 717 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, the media, local law enforcement, and posted substitute notice on its website. Following the breach incident, the CE sanctioned the workforce member and implemented additional administrative and technical safeguards and retrained its staff on safeguarding PHI.
