OCR description
The covered entity (CE), Child Focus, reported that it experienced a cyberattack affecting the protected health information (PHI) of 2,716 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, health insurance information, and diagnoses. The CE notified HHS, affected individuals, and the media. Following the breach, the CE offered free credit monitoring and identity protection services and strengthened its administrative, technical, and security safeguards.
