Miracle-Ear

The covered entity (CE), Miracle-Ear, reported that its facility was burglarized and the protected health information (PHI) of 484 individuals was compromised. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. A toll-free phone number was established for questions and concerns. In its mitigation efforts, the CE implemented additional physical safeguards to better protect its PHI.
