Assistcare Home Health Services LLC dba Preferred Home Care of NY/Preferred Gold

The covered entity (CE), Assistcare Home Health Services, LLC, dba Preferred Home Care of New York/Preferred Gold, reported that it was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 57,345 individuals. The ePHI involved included names, addresses, dates of birth, clinical information, diagnoses, laboratory results, medications prescribed, treatment information, and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE strengthened its technical and security safeguards to better protect its ePHI. The CE also retrained its staff.
