OCR description
The covered entity (CE), Sleep Medicine Associates of Texas, reported that a document containing the names of 994 individuals was disclosed to an unauthorized individual. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE sanctioned the workforce member involved and re-educated its workforce members on safeguarding protected health information.
