OCR description
The covered entity (CE), Illinois Department of Healthcare and Family Services, reported that an employee mailed the protected health information of 8,848 individuals to the wrong recipients. The PHI involved included names, dates of birth, addresses, social security numbers, financial information, diagnoses, conditions, citizenship status, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional administrative and technical safeguards. OCR provided technical assistance regarding the HIPAA Rules.
