OCR description
The covered entity (CE), Health Net Community Solutions, reported that its business associate (BA) experienced a cyber-attack that affected the electronic protected health information (ePHI) of 688,603 individuals. The ePHI involved included names, addresses, dates of birth, health insurance information, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Following the breach, the CE offered one year of complimentary credit monitoring services to affected individuals and terminated its business relationship with the BA.
