OCR description
The covered entity (CE), Health Net Life Insurance Company, reported that its business associate (BA) experienced a cyber-attack that affected the electronic protected health information (ePHI) of 30,842 individuals. The ePHI involved included names, addresses, dates of birth, health insurance information, and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Following the breach, the CE offered complimentary credit monitoring services and terminated its business relationship with the BA.
