OCR description
The covered entity (CE), Trusted Health Plans, reported that it was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 200,665 individuals. The ePHI involved included names, addresses, Social Security numbers, dates of birth, diagnoses/conditions, lab results, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE strengthened its technical safeguards to better protect its ePHI.
