OCR description
The covered entity (CE), BioTelemetry, Inc., reported that a subcontractor of its business associate was the victim of a cyber-attack that compromised the protected health information (PHI) of 38,575 individuals. The PHI involved included names, dates of birth, addresses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media.
