OCR description
The covered entity (CE), Belden, reported that it experienced a cybersecurity incident affecting the protected health information (PHI) of 9,840 individuals. The PHI involved included names, dates of birth, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, the media, and law enforcement. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional technical and administrative safeguards to better protect its PHI. OCR provided technical assistance to the CE.
