Maestro Health, LLC

The business associate (BA), Maestro Health, reported that several employees were the victims of an email phishing attack affecting the protected health information (PHI) of 18,488 individuals. The PHI involved included names, dates of birth, addresses, and Social Security numbers. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional administrative, technical, and security safeguards, revised its policies and procedures, retrained its employees on email security, and provided affected individuals with credit monitoring services. OCR provided technical assistance to the BA regarding the HIPAA Rules.
