OCR description
The covered entity (CE), Harrington Physician Services, reported that one of its employee’s inadvertently uploaded a file on the CE’s server that was accessible by the public. This breach affected the electronic protected health information (ePHI) of 4,393 individuals. The ePHI involved included names, addresses, dates of birth, and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE removed the files from the server.
