OCR description
The covered entity (CE), Neighborhood Healthcare, reported that its business associate (BA) experienced a ransomware attack that affected the electronic protected health information (ePHI) of 45,200 individuals. The ePHI involved included names, addresses, Social Security numbers, diagnoses, treatment information, and claims information. The CE notified HHS, affected individuals, and the media. The CE also ended its business relationship with the BA.
