OCR description
The covered entity (CE), The Texas MedClinic, reported that the protected health information (PHI) of 6,638 individuals was accessible and viewable on iPads used by patients during the registration process. The PHI involved included names, dates of birth, addresses, and Social Security numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
