OCR description
The covered entity (CE), Virtua Health, Inc., reported that it inadvertently sent documents containing the protected health information (PHI) of 2,704 individuals, to their former addresses. The PHI involved included names, Social Security numbers, claims information, dates of services, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
