OCR description
The covered entity (CE), Kanan Medical, reported that two employees took and misappropriated the protected health information (PHI) of 823 individuals. The PHI involved included names and other demographic information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI.
