Achievement Center of LECOM Health

The covered entity (CE), Achievement Center of LECOM Health, reported that an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 1,671 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses/conditions, lab results, medications prescribed, and financial and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect its PHI.
