OCR description
The covered entity (CE), SummaCare, reported that its business associate (BA) mailed envelopes containing the protected health information (PHI) of 5,532 individuals, to the wrong individuals. The PHI involved included names and addresses. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE severed its relationship with the BA and updated its HIPAA Breach Notification Rule policy.
