OCR description
The covered entity (CE), Tyler Family Circle of Care, reported that its business associate (BA) was subject to a ransomware attack that affected the electronic protected health information (ePHI) of approximately 1,860 individuals. The ePHI involved included names, addresses, birthdates, and medications prescribed. The BA notified affected individuals and the media while the CE notified HHS.
