OCR description
The covered entity (CE), Ascension St. Joseph Hospital, reported that its business associate (BA) experienced a hacking incident that affected the electronic protected health information (ePHI) of 5,807 individuals. The ePHI involved included names, dates of birth, diagnoses/conditions, medications prescribed, treatment information, financial information, and health insurance information. The CE notified HHS, affected individuals, the media, and provided substitute notice.
