The covered entity (CE), Exceltox Laboratories, reported that a shipping company lost its documents that contained the protected health information (PHI) of 4,571 individuals. The PHI involved included names, Social Security numbers, addresses, dates of birth, lab results, and claims information. The CE notified HHS, affected individuals, and the media.
