OCR description
United Health Group Health Plan, the covered entity (CE), reported that a network disruption caused the protected health information of 666 individuals to be viewable by others. The PHI involved included names, addresses, and claims information. The CE notified HHS and the affected individuals. In its mitigation efforts, the CE implemented additional administrative safeguards and provided complimentary credit monitoring services to affected individuals.
