OCR description
The covered entity (CE), SAC Health Systems, reported that its business associate (BA) experienced a ransomware attack that affected the electronic protected health information (ePHI) of 28,128 individuals. The ePHI involved included names, addresses, Social Security numbers, financial and health insurance information, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE terminated its business relationship with its BA.
