OCR description
The covered entity (CE), Trinity Health System – Twin City, reported that a business associate (BA) experienced a hacking incident that affected the electronic protected health information (ePHI) of 9,579 individuals. The ePHI involved included names, dates of birth, medications prescribed, and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice.
