OCR description
The covered entity (CE), Washoe Barton Medical Clinic dba Carson Valley Medical Center, reported that its business associate experienced a cyber-attack that affected the electronic protected health information (ePHI) of 1,115 individuals. The ePHI involved included names, addresses, dates of birth, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media.
