OCR description
The business associate (BA), Dean Health Servicing Company, reported that an employee of its subcontractor was the victim of an email phishing attack that compromised the protected health information (PHI) of approximately 1,025 individuals. The PHI involved included names, addresses, email addresses, dates of birth, Social Security numbers, claims and health insurance information, and other treatment information. The BA notified HHS, affected individuals, the media, and posted substitute notice on its website.
