OCR description
The covered entity (CE), Arkansas Health and Wellness Health Plan, reported that its business associate was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 3,627 individuals. The ePHI involved included names, dates of birth, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media.
