OCR description
The covered entity (CE), SEIU 775 Benefits Group, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 140,000 individuals. The PHI involved included names, addresses, and Social Security numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice. Following the breach incident and OCR’s investigation, the CE implemented new administrative and technical safeguards.
