OCR description
The covered entity (CE), Monroe County Health Center, reported that its business associate (BA) was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 757 individuals. The ePHI involved included names, dates of birth, prescription information, and medical record numbers. The CE notified HHS and the BA notified affected individuals and the media.
