OCR description
The covered entity (CE), Hill Country Community Clinic dba Hill Country Health and Wellness Center, reported that its business associate experienced a cyber-attack that affected the electronic protected health information (ePHI) of 2,658 individuals. The ePHI involved included names, dates of birth, and medications prescribed. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website.
