Spire Power Solutions, L.P.

The covered entity (CE), Spire Power Solutions, reported that an employee of its business associate (BA) experienced an email phishing attack that affected the electronic protected health information (ePHI) of 771 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, health insurance information, and treatment information. The BA notified HHS, affected individuals, and the media. In response to the breach, the CE updated its policies and procedures to provide additional safeguards for its ePHI. OCR provided technical assistance to the CE regarding business associate agreements and obtained assurances that the CE implemented the corrective actions noted.
