OCR description
The covered entity (CE), East Jordan Family Health Center, reported that its business associate (BA) experienced a hacking incident that affected the protected health information (PHI) of 1,146 individuals. The PHI involved included names, dates of birth, medications, and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE and BA implemented additional administrative and technical safeguards and provided complimentary credit monitoring services to affected individuals.
