OCR description
The covered entity (CE), San Juan Regional Medical Center, reported that it was the victim of a ransomware attack that affected the protected health information (PHI) of 325,660 individuals. The PHI involved included names, dates of birth, Social Security numbers, drivers’ license numbers, financial and health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards, and revised and created new policies and procedures.
