Discovery Practice Management, a business associate (BA), reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 12,859 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, drivers’ license numbers, medical record numbers, diagnoses, prescription information, and health insurance and financial information. The BA notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the BA implemented additional administrative and technical safeguards and retrained it staff.
