OCR description
The covered entity (CE), Glacier Medical Associates, reported that it experienced a ransomware attack affecting the protected health information (PHI) of 3,007 individuals. The PHI involved included names, dates of birth, addresses, diagnoses, and conditions. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical safeguards and revised its policies and procedures. OCR provided technical assistance to the CE regarding the HIPAA Security Rule.
