OCR description
The covered entity (CE), PA Health & Wellness, reported that its business associate (BA) was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 1,325 individuals. The ePHI involved included names, addresses, dates of birth, health insurance information, medical conditions, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE strengthened its technical safeguards to better protect its ePHI.
