OCR description
The covered entity (CE), UC San Diego Health, reported that an employee was subjected to an email phishing scheme that compromised the protected health information (PHI) of 495,949 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license and Social Security numbers, claims and financial information, lab results, medications, diagnoses, and other treatment information. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. Staff were retrained on email security.
