OCR description
The covered entity (CE), Falcon Group International Corp., reported that an employee impermissibly accessed the protected health information (PHI) of 1,900 individuals. The PHI involved included names, addresses, dates of birth, and other identifiers. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE sanctioned the employee involved and implemented additional administrative, technical, and physical safeguards to better protect its PHI.
