OCR description
The covered entity (CE), Neighborhood Healthcare, Inc. dba Neighborhood Family Practice, reported that its business associate (BA) experienced a cyber-attack that affected the electronic protected health information (ePHI) of 6,156 individuals. The ePHI involved included names, dates of birth, medication information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website.
