OCR description
The covered entity (CE), Golden Care Pharmacy, reported that an employee impermissibly accessed and altered the protected health information (PHI) of 5,530 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, medications, and claims information. Subsequently, the Department of Justice (DOJ) announced that the owner was arrested and charged by indictment with conspiracy to commit heath care fraud and four counts of health care fraud. In light of the DOJ’s pursuit of criminal charges, OCR closed its investigation.
