OCR description
The covered entity (CE), Little Hill Foundation for the Rehabilitation of Alcoholics dba Alina Lodge, reported that its business associate experienced a ransomware attack affecting the electronic protected health information (ePHI) of approximately 2,565 individuals. The ePHI involved included names, addresses, phone numbers, dates of birth, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice.
