Eye Consultants of Atlanta / Piedmont System Ambulatory Surgery Center, LLC

The covered entity (CE), Eye Consultants of Atlanta / Piedmont System Ambulatory Surgery Center, reported that medical records that were marked and separated for shredding were inadvertently disposed of by janitorial staff. The breach involved the protected health information (PHI) of 2,662 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and diagnoses. The CE notified HHS, affected individuals, the media, and offered complimentary credit monitoring services. In its mitigation efforts, the CE retrained its workforce members on the proper procedures for disposal of medical records and other sensitive data.
