Hosparus, Inc. dba Care Guide Partners Inc.

The covered entity (CE), Hosparus, Inc. dba Care Guide Partners, Inc., reported that a subcontractor of its business associate (BA) experienced a ransomware attack that affected the electronic protected health information (ePHI) of 1,545 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also provided complimentary credit monitoring services. In its mitigation efforts, the BA implemented additional technical safeguards to better protect its sensitive data.
