OCR description
The covered entity (CE), Abode Services, reported that several employees were victims of an email phishing attack affecting the protected health information (PHI) of 4,905 individuals. The PHI involved included names, addresses, treatment information, and financial information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE retrained its workforce members on email security and implemented additional technical safeguards.
