John Muir Physician Network

The covered entity (CE), John Muir Physician Network, reported that an employee impermissibly provided an individual with access to an Excel file that contained the electronic protected health information (ePHI) of 1,081 individuals. The ePHI involved included names, diagnoses, and health insurance and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE sanctioned the responsible employee and implemented additional administrative and technical safeguards to protect its sensitive data.
